lack of accountability,
Profit and convenience considered more important than safety and common sense.

There is a tab on this site called 'Bhopal, India - Chemical" that show another example of an avoidable Industrial Disaster..

The key is you must run the corporation in a way that includes safety and required maintenance..
If you take the money that should be reinvested as maintenance, and provide it as short term profit to shareholders...  then someone else (or the environment) suffers the consequences.........

Could be that the operating model needs to be revisited....  (do you think?)
